This session by Medical Business Services will provide an overview of the new GP Chronic Condition Management Plan (GPCCMP) framework. The GP Chronic Condition Management Plan (GPCCMP) replaced the former GP Management Plans (GPMPs) and Team Care Arrangements on 1 July 2025. The session also covers updates to the referral process, what GPs are doing differently and how this impacts allied health practice and the transition arrangements and billing implications.
