Primary Health Tasmania’s Shared Transfer of Care program sets the standard for transfers of care by promoting a shared approach between organisations, service providers, communities and healthcare consumers.
This website aims to support health professionals working with people with chronic and complex conditions to acquire the knowledge and skills to implement quality shared transfers of care.
It incorporates the Shared Transfer of Care eLearning course and offers a range of useful resources.
