The Education Interface Program - Heart failure management.

A one-and-a-half-hour webinar about heart failure management, organised by the Cardiac Community Network and supported by Primary Health Tasmania in partnership with the Tasmanian Health Service GP Liaison Officers.

The Cardiac Community Network has been set up with the aim to improve communication and collaboration between primary and secondary care services, focusing on the management of cardiac related conditions.

This webinar covers:

Heart failure, which affects 2% of the adult Australian population and 10% of those over the age of 75 years.

General practitioners are certainly going to encounter patients with a diagnosis of heart failure. This syndrome can be seen with preserved ejection fraction (HFpEF) or reduced ejection fraction (HFrEF), which have equal prevalence but different management approaches.

Morbidity and mortality are high with frequent re-admissions for decompensated heart failure. We will explore strategies to improve heart failure outcomes and standardise the continuum of care between primary and secondary healthcare facilities.
